Health & Wellbeing

Our Wellbeing Hub helps service user manage their finances to better support their long-term health.

Foundation 92 delivers the Wellbeing Hub for people on probation in Stockport to support and motivate them to make positive changes.

The service is partnership led by The Big Life Group comprising of 11 local Greater Manchester organisations. The mission is to make sure that people have all the tools, information and support they need whilst on probation.

The Wellbeing Hub helps support service users with their physical, emotional and mental wellbeing, family and relations, building social connections, money management skills, access to training and employment and empowering people to take the best possible steps to move forward in their lives.

“Through supporting the client with their initial referral reason, it became clear that additional support around finances would be beneficial. The client has learning needs but, due to being unaware of available help, has gone through life without the appropriate support.

This has significantly affected their self-confidence, ability to socialise, and capacity to manage finances. Last year, the client was involved in a workplace accident, resulting in serious physical health issues.

Being self-employed, they were left facing financial hardship, new health needs, and poor housing conditions. The client was referred to the Wellbeing Hub in January 2025 to explore housing options.

Since then, they have been supported to register for social housing. Stockport Homes was informed of the client’s difficulty navigating the online system, so they arranged alternative support— allowing bids via phone or in person.

The client was also supported with their Universal Credit claim, including applying for LCW, due to multiple upcoming surgeries, daily pain, and difficulties attending appointments or using the journal.

Further support included help applying for PIP (now awarded), a concessionary travel pass, clothing and food bank vouchers.

The client successfully achieved their goal of exploring housing and now has financial support to better manage their health long term.”

The Wellbeing Hub exists to prevent and reduce re-offending, recall rates and crime in the community. People are supported using a person centred, strength based and trauma informed approach to improve their wellbeing and circumstances.
